This is the work permit application form you will need when you are applying for a job. Please print out both pages. The second page has the information regarding ages and the hours you are allowed to work.
Once this form is completed, please email Ms. Morgan, lmorgan@pgusd.org for an appointment to turn in the application and receive your actual work permit.
